Troy’s extensive personal experience in the field of recovery has driven him to help others. His life and career has been built on passion and an endless love for others. He has a drive for success that is rarely rivaled. He personally takes the time to engage with every client that arrives at Deer Hollow Recovery Centers. Troy loves his beautiful wife and family. He enjoys fly fishing, fast cars and serving his community. Troy is memorable, a Father figure to many, and a true gentleman to all.
Genevieve has been in the healthcare industry for 12 years, with a majority of that focused in behavioral health. She received her degree in social work from Oregon State University. Genevieve has a unique devotion to help people. She has held various positions nationally with marketing, program development, and hospital administration. Genevieve eventually partnered on developing a residential military program to promote and educate active duty military and their families. This is where she found fulfillment working with individuals suffering from trauma and addiction. Genevieve is now using her experience to work for a company that aligns with her passion, integrity and loyalty to help those in need.
Monica attended the University of New England College of Osteopathic Medicine and graduated in June 2003. After school, she completed her Adult Psychiatry Residency at the Institute of Living in Hartford, Connecticut and Thomas Jefferson University Hospital in Philadelphia, Pennsylvania. Soon after, Monica completed a Child and Adolescent Psychiatry Fellowship at Children’s National Medical Center in Washington, DC. During her last year of training, she completed a Forensic Psychiatry Fellowship at the University of Maryland Medical Center. Monica had always wanted to be a psychiatrist because of her belief in the importance of mental health, and she found psychiatry to be an extremely interesting area of medicine. Dr. Polk’s true desire is to contribute to the recovery of each individual she treats at Deer Hollow Recovery. In her free time, she enjoys going for jeep rides in the mountains and spending time with her family, including her two poodles.
Erica graduated with honors from Utah Valley University with a Bachelor’s of Science in Nursing. She also graduated with honors from the University of North Florida with a Bachelor’s of Science in Athletic Training. Although Erica has experience working with various populations, her specialty lies with those suffering from mental illness and/or addiction.
Erica developed a passion caring for individuals who struggle with behavioral health early on in her career. She is excited to be working in a setting where she can help guide those individuals to a life of recovery, as well as contribute her experience in state licensing and Joint Commission accreditation.
Amy Crawford, PhD, LMFT has been in the field of mental health and addiction medicine for the last 16 years. Throughout this journey, trauma has become her passion and specialty. She has spent the majority of her career treating the invisible wounds of trauma in varied settings with a focus on Military Service Members and First Responders. Amy has had the honor of witnessing over 4,000 trauma accounts. She is certified in Cognitive Processing Therapy, Prolonged Exposure, EMDR, MBSR and is a Registered Yoga Teacher. Amy is in the process of getting her Doctor of Traditional Naturopathy to better understand how natural healing; herbal medicine and nutrition can play an integral role in helping mental wellness. Amy is wildly passionate about the work she gets to do and loves holding the hope for others while they step into their healing and recovery.

Sloane is an LCSW that’s been in the field since 2009. She’s been working in mental health however since 2004. She received her degree from the University of Utah in social work. Her main focus in dual-diagnosis treatment. She’s worked with children through the geriatric, but has chosen to focus mainly on adult populations. She has been focused primarily on outpatient where she’s helped to create three IOP programs.

Matt Quackenbush, LCSW is an experienced therapist and mental health advocate with over 12 years of experience and practice in the treatment of trauma and addiction, working in mental health field since 2007. A University of Utah alum, Matt’s extensive training and experience include expertise in the most cutting edge treatment modalities EMDR, Mind Body Bridging, CPT, DBT, and ACT. Matt is a husband and father to four children. He is passionate about fitness, health and nutrition, a Level 1 CrossFit Trainer. Most importantly though, Matt has dedicated his life to learning about and treating trauma and addiction, leading his clients through their journey with his unique passionate, empathic approach.
